Why drivers miss MOT and tax deadlines:-

The honest answer is that the system barely prompts anyone.

Vehicle tax at least triggers a V11 letter. Insurance generates emails, texts and usually a phone call. The MOT gets nothing automatic unless you actively hunt down a reminder service and most drivers never learn one exists.

Yes. You cannot tax a vehicle in Great Britain without a valid MOT, which is exactly why watching both dates together is more useful than watching either alone.
